IF YOU HAVE ANY OF THE FOLLOWING SYMPTOMS PLEASE DO NOT ENTER THE HEALTH CARE CENTRE
• Fever, new onset of cough, worsening chronic cough, difficulty breathing, shortness of breath, sore throat, trouble swallowing, congestion or runny nose without cause, Pink eye
• Decrease or loss of taste or smell, nausea, vomiting, diarrhea, abdominal pain, fatigue, extreme tiredness, unexplained muscle aches or joint pain or generally not feeling well
• Flu like symptoms
PLEASE DO NOT ENTER IF:
You have tested positive for Covid-19 in the last 10 days
You are awaiting Covid -19 test results for any reason (i.e. symptoms, exposure to a positive case, medical appointment, regular screening for work or visits to long term care)
You or any of your household members have needed to be isolating because of symptoms, exposure to a positive case or a positive Covid-19 test result within the last 10 days
You have been in close contact with a person who has a confirmed or probable case of Covid-19 in the last 10 days
Please delay and reschedule your visit, contact your health care
provider, the Thunder Bay District Health Unit or call 1-866-797-0000 for more information.
IF YOU ANSWERED NO TO ALL OF THE ABOVE PLEASE ENTER
THE CLINIC AT THE SCHEDULED TIME OF YOUR APPOINTMENT.
• Upon entry your hands will be sanitized or you will be asked to wash your hands
• You will be asked to read and sign a Covid-19 screening questionnaire
• You will be required to wear a mask at all times as they are mandatory, we recommend you wear surgical grade masks rather than cloth masks for your added protection
• Please follow the social distancing signage that is on the floor
and overhead in our waiting area and hallways
• Please leave all unnecessary items in your vehicle
• Companions are only allowed if you are accompanying a minor, patients only please
